Job summary

Hospice of the Midwest in Edina, MN is seeking a passionate Hospice RN Case Manager to join our caring team. You will provide expert, patient-centered care at a crucial time, delivering 1:1 support to patients and families in a supportive, fulfilling environment.

The role offers flexible scheduling, opportunities for professional growth, and a collaborative team atmosphere. You will work in a home-based hospice setting with a strong emphasis on compassionate care.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management; educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
