Job summary

Hospice of the Midwest in Woodbury, MN is seeking an On-Call Registered Nurse to join our team. This role involves providing after-hours hands-on hospice care, collaborating with the hospice team, and supporting patients and families in a compassionate, professional manner.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Collaborate with the on-call team to deliver hands-on hospice care during after-hours.
• Deliver hands-on care to hospice patients and support their families during on-call hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols or orders.
• Communicate changes in condition to the physician and obtain orders as needed.
• Provide guidance to patient and family regarding condition and next steps.
• Pronounce death when applicable during on-call hours and assist family and hospice team.
• Complete timely documentation and communication.

Schedule:Mon-Thu, Overnight Shift (5:00 PM - 8:00 AM)

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication
