Job summary

Hospice of the Midwest in Woodbury, MN seeks a compassionate Hospice Admissions RN to join our team. You will visit patients in homes, hospitals and facilities, discuss goals and care with patients and families, obtain consents and coordinate the start of services with the interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured and in good working order and CPR certification is required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.

As a Hospice Admissions RN You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required , 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.

Salary Range

USD $92,000.00 - $95,000.00 / Year
