Job summary

Hospice of North Alabama is seeking a dedicated Registered Nurse Case Manager to provide patient-centered hospice care in the Decatur/Cullman area. You will assess needs, develop care plans, administer treatments, and support families with empathy.

This role offers a collaborative team environment with on-call rotation, flexible scheduling options, and opportunities for professional growth within a growing organization that values compassionate care.

Qualifications

• Graduate of an accredited nursing school with current state licensure

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation

• One year nursing experience required, 3+ years preferred

• Experience in hospice or a similar setting preferred

• Knowledge of the hospice philosophy of care

• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
