Job summary

Hospice of North Alabama is seeking an on-site RN Case Manager to provide compassionate, patient-centered hospice care in the Decatur area. You will assess needs, develop and implement care plans, administer treatments and work with an interdisciplinary team to meet patient goals.

The role requires an RN license, 1+ year nursing experience (3+ preferred), and a commitment to clinical excellence. On-site schedule is Monday–Friday, 8am–5pm with on-call rotation as needed.

Qualifications

• Graduate of an accredited nursing school with current RN licensure.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Valid driver's license and insured vehicle

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentationParticipate in on‑call rotation as required by local branch
