Hospice of North Alabama in Decatur, AL is seeking a passionate RN Case Manager to join our team. You will deliver patient-centered hospice care and collaborate with an interdisciplinary team to honor patients' goals.

Requirements include graduation from an accredited nursing school, current Alabama RN licensure, and at least 1 year nursing experience (3+ preferred). We offer comprehensive benefits and a supportive work environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a reliable vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:
