Job summary

Hospice of North Alabama seeks an experienced Registered Nurse Case Manager to provide patient-centered hospice care in Union Grove, AL. This role covers the East Morgan county area with a Monday–Friday schedule, 8am–5pm, and opportunities to work independently with team support.

You will assess patient needs, develop care plans, deliver direct nursing care, and educate families while maintaining documentation and collaborating with the interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in-state
• One year nursing experience required; 3+ years preferred
• Experience in hospice or a similar setting preferred
• Licensed driver with insured vehicle
• CPR certification maintained as required

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligned with patient wishes and goals
• Direct nursing care: administer medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
