Job summary

Hospice of North Alabama is seeking an experienced RN Case Manager to join our team in Decatur, AL, with an office in Cullman, AL. You will provide patient-centered hospice care, assess needs, create care plans, administer treatments, and support families in a compassionate environment.

Qualifications

• Must hold valid RN license in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
