Hospice of North Alabama is seeking an experienced Registered Nurse Case Manager to join our team in Decatur, AL. You will provide patient-centered hospice care, develop individualized care plans, and collaborate with an interdisciplinary team to support patients and families.

Responsibilities include assessing needs, administering medications, managing symptoms, documenting care, and participating in on-call rotation. A valid state nursing license and driving with insured vehicle are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

As a RN Case Manager You will:
