Hospice of North Alabama is seeking a dedicated RN Case Manager in Huntsville, AL to deliver patient-centered hospice care. The role emphasizes assessment, planning, direct nursing care, symptom management, and family education within a supportive team.

The position requires an accredited nursing degree, active RN licensure, and 1+ year of nursing experience; hospice experience is preferred, with opportunities for professional growth and stability.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements.

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients' wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:
