Job summary

Hospice of North Alabama in Florence, AL is seeking a compassionate Registered Nurse to join our team as an RN Case Manager. You will assess physical, emotional, and spiritual needs, develop and monitor care plans, administer medications, and provide pain and symptom management in a home-based setting.

This role offers flexible scheduling, opportunities for growth, and the support of an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Alabama.
• Hospice or palliative care experience preferred.
• CPR certification required.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation

• One year nursing experience required, 3+ years preferred

• Experience in hospice or a similar setting preferred

• Knowledge of the hospice philosophy of care

• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
