Job Description

ProMedica Toledo Hospital, one of the largest acute-care facilities in the region, with 794 beds and over 4,800 healthcare professionals serving northwest Ohio and southeast Michigan, is seeking an RN / Acute Care Navigator. The role involves coordinating and ensuring continuity of care through effective transitions of care management as patients move in and out of the acute care environment.

Position Summary

RN, Acute Care Navigator, ensures coordination and continuity of care through effective transitions of care management as patients move in and out of the acute care environment.

Job Requirements

1. Active Registered Nurse licensure in the state of practice with BSN or 2 years of applicable nursing experience.
2. Positive rapport with patients, families, physicians, and interdisciplinary team; experience in patient advocacy and knowledge of hospital and community resources.
3. Experience working with diverse populations.
4. Exceptional problem-solving, critical-thinking, organizational, interpersonal, and communication skills.
5. Ability to work in a self-directed environment with attention to detail and follow-through.
6. Ability to function effectively in a critical care environment.
7. Ability to establish priorities and respond to inquiries.
8. Physical ability to stand for long periods and work rapidly for extended periods.
