Qualifications

• Active RN licensure in good standing in the applicable state.
• BLS certification is required.
• Experience with Electronic Medical Records (EMR) systems.
• US work authorization is required.
• Ability to assess patients without face-to-face interaction and communicate effectively.

Responsibilities

• Provide competent nursing care following established protocols and safety standards.
• Respond to incoming requests with reliable, timely communication.
• Deliver triage and symptom management for patients both in person and remotely.
• Utilize standardized medication management and prior authorizations when needed.
• Review patient medications and update records as required.
• Provide patient education and direct care for chronic conditions, via in-person, phone, or group sessions.
• Collaborate with care team to monitor progress of care plans.
• Build relationships with patients and caregivers to support preventative care.
• Create a welcoming environment and delegate tasks to team members as needed.
• Participate in care team meetings to discuss patient care and clinic operations.
• Deliver exceptional patient experience through service, responsiveness, and respectful care.
• Perform point-of-care testing and specimen collection as needed.
• Perform other related duties as assigned.
