Job summary

UnitedHealth Group's Optum Home & Community Care is seeking a registered nurse (RN) to join the Senior Community Care team in Fayetteville, NC. The RN will perform patient assessments, collaborate with the interdisciplinary care team, and support care coordination for high-risk members in facility and home settings.

This role requires an RN license, BLS, ability to lift 30 pounds, and reliable transportation for home visits.

Qualifications

• Associate degree or higher in Nursing (RN) is required.
• Active and unrestricted RN license in the state of residence or ability to obtain by start date.
• Certification in Basic Life Support (BLS) required.
• Ability to perform physical assessments and move a 30-pound bag.
• Access to reliable transportation for home visit assessments.

Responsibilities

• Ensure accurate and complete patient assessments.
• Communicate and collaborate with the interdisciplinary care team.
• Assist the provider/team with care coordination activities and benefit determinations.
• Coordinate training and quality measures.
• Document and code accurately in charts and EMR.
