Job summary

Your Health Organization is actively seeking a Registered Nurse to join our growing urgent care and home care team. You will service patients in South Carolina and Georgia, coordinating care with providers across homes, clinics, and assisted living facilities.

You will collaborate with specialists to ensure seamless, quality care and participate in telehealth and care planning activities. The role requires strong clinical experience, leadership, and communication skills, with responsibilities

Qualifications

• Must be a licensed nurse in good standing.
• Minimum of three years clinical experience preferred.
• Ability to supervise professional and administrative personnel.
• Strong reading, communication, and leadership skills.

Responsibilities

• Coordinate care for patients in homes and assisted living facilities.
• Collaborate with care teams and physicians to meet quality measures.
• Facilitate telehealth visits and coordination of care.
• Maintain up-to-date patient records in the EMR.
