Job summary

Oak Street Health in High Point, NC is seeking a dedicated Registered Nurse to coordinate patient care and deliver competent nursing services in a welcoming clinic setting. You will assess patients, perform procedures within scope, triage requests, and collaborate with physicians and the care team to ensure timely, high-quality outcomes.

This full-time, on-site role includes using EMR systems, educating patients on chronic conditions such as diabetes, supporting care plans, and maintaining HIPAA

Qualifications

• Active RN license in North Carolina in good standing.
• Current CPR and/or BLS certification.
• Experience using Electronic Medical Records (EMR).
• Clinic setting experience with vitals, injections and vaccines.
• Ability to maintain patient confidentiality and HIPAA compliance.
• Strong communication and adaptability in a busy clinic.

Responsibilities

• Provide competent nursing care in a safe, timely manner.
• Respond to phone requests and support patient needs.
• Triage and manage symptoms clinically for in-person or remote care.
• Use protocols for medications, refills, and prior authorizations.
• Review patient medications and update records accurately.
• Educate patients on chronic conditions and self-management.
• Collaborate with care team to monitor progress and care plans.
• Support patient and caregiver relationships and equitable care.
