Wellstar Health System is seeking a Preventative Health Navigator who is a registered nurse with preventative health knowledge. The role provides education and resources to help patients access screenings, surveillance, and care coordination services.

You will work with a multidisciplinary team to promote timely care, remove barriers, and support population health initiatives. Ideal candidates have 5+ years of nursing experience, GA RN licensure (or compact), and BLS certification.

Qualificações

• Associates degree in Nursing or Diploma in Nursing; Bachelors preferred.
• RN license with GA or multi-state compact.
• BLS certification required (Basic Life Support).
• Minimum 5 years of nursing practice; oncology, pulmonary, thoracic, colorectal or outpatient case management experience preferred.

Responsabilidades

• Perform the Nursing Process to assess, plan, implement, and evaluate patient care.
• Coordinate delivery and documentation of safe, high-quality care within an interdisciplinary team.
• Support growth and development of the population health team and navigation program.
• Participate in data collection and continuous improvement activities.
