Job summary

Wellstar Health System in Georgia seeks a Preventative Health Navigator, a registered nurse who supports patients, families and caregivers by improving access to screenings, resources and care coordination. You will apply the nursing process to educate and empower patients to overcome barriers and navigate complex health services, in person, by phone, or electronically.

Qualifications

• Associates degree in nursing or diploma; bachelor’s preferred.
• RN licensure GA or Compact required; BLS certification required.
• Minimum 5 years of nursing experience; oncology or related fields preferred.

Responsibilities

• Perform nursing process and coordinate care with an interdisciplinary team.
• Improve access to care by identifying barriers and resources.
• Educate patients and families to enable informed decisions.
• Support population health initiatives and performance improvement.
• Document care and collaborate with program leaders and administration.
