Wellstar Health System in Georgia seeks a Preventative Health Navigator, a registered nurse focused on population health and care coordination. You will work with patients, families, and a multidisciplinary team to remove barriers to care and promote timely access to quality screenings, surveillance, and care coordination services.

This role emphasizes education, resource navigation, and evidence-based practice, interfacing with patients in person, by phone, or electronically.

Qualifications

• Associates Nursing or Diploma in Nursing; BSN preferred.
• RN license in good standing; active BLS required.
• Minimum 5 years of nursing practice; Oncology, pulmonary, thoracic, colorectal, or outpatient case management experience preferred.

Responsabilités

• Performs the Nursing Process with evidence-based practice and coordinates care through interdisciplinary partnerships.
• Coordinates delivery and documentation of safe, quality patient care with diverse communication.
• Participates in growth and development with the population health team and supports navigation across services.
