Wellstar Health System in Georgia is seeking a Preventative Health Navigator who is a registered nurse with preventative health knowledge. You will provide education and resources to patients, families, and caregivers to improve access to screenings and care coordination.

As a member of a multidisciplinary team, you will identify barriers to care, coordinate with clinics, and promote population health through evidence-based practice and proactive navigation across services.

Qualificações

• Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred.
• BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-Instructor.
• RN license (Single State) or RN-COMPACT - RN - Multi-state Compact
• Speciality disease or role specific area preferred
• Minimum 5 years of nursing practice; Oncology, pulmonary, thoracic, colorectal, or outpatient case management experience preferred
• Ability to read, write and speak English; strong communication and time management skills

Responsabilidades

• Performs the Nursing Process (assessment; diagnosis; outcomes; planning; implementation and evaluation) in evidence-based practice.
• Coordinates the delivery and documentation of safe, quality patient care with effective interdisciplinary communication.
• Demonstrates teamwork and collaboration while promoting fiscal responsibility and workforce retention.
• Participates in performance improvement and supports shared governance at all levels.
• Coordinates care at specialty clinics and multidisciplinary rounds; identifies gaps and bridges resources.
• Improves patient care timeliness and access; collaborates with leadership on population needs assessments.
