The Preventative Health Navigator at Wellstar Health System is a registered nurse focused on removing barriers to care and coordinating services for patients, families, and caregivers. The role uses the nursing process to guide education and timely access to screenings and care coordination services.

You will work with a multidisciplinary team, including doctors and other clinicians, to support positive patient health outcomes and promote population health initiatives across Wellstar’s network.

Qualifikationen

• Associates degree or diploma in nursing; BSN preferred.
• Active RN license in Georgia.
• BLS certification required.
• Minimum 5 years of nursing experience; oncology, pulmonary, thoracic, colorectal, or outpatient case management experience preferred.

• Performs nursing process with evidence-based care and coordinates care through partnerships.
• Ensures safe, timely documentation and promotes an inclusive clinical environment.
• Collaborates with navigation team to identify gaps and coordinate high-level care across clinics and services.
• Participates in data collection and population health initiatives to improve access and outcomes.
