Qualifications

• Minimum 5 years of nursing practice.
• Oncology, pulmonary, thoracic, colorectal or outpatient case management experience preferred.
• Experience using health technology and care coordination tools.

Responsibilities

• Perform nursing process to assess, plan, implement and evaluate patient care.
• Coordinate care with multidisciplinary teams to improve timeliness and access.
• Identify barriers to care and connect patients with resources.
• Support performance improvement and evidence-based practice.
