Job summary

Wellstar Health System seeks a Preventative Health Navigator (RN) to support patients, families and caregivers in overcoming access barriers and coordinating care. You will educate, guide and connect patients with screenings, resources and services across a multidisciplinary team, with an emphasis on equity and quality outcomes.

This role interfaces with clinicians and support staff in person, by phone or electronically, focusing on timely access to preventive care and population health

Qualifications

• Minimum 5 years of nursing practice, oncology, pulmonary, thoracic or related areas preferred.
• Registered Nurse licensure in Georgia or multi-state compact.
• BLS certification required.
• Graduate-level population health or care coordination experience is a plus.

Responsibilities

• Implements the nursing process to assess, plan, coordinate and evaluate patient care.
• Identifies barriers to care and connects patients with community resources.
• Documents and communicates care plans across multidisciplinary teams.
• Supports population health initiatives and care coordination across the Wellstar network.
