Job summary

Advocate Health is seeking an RN Navigator for the Wake Forest Baptist Medical Center - Spine Center in Clemmons, NC. The role focuses on coordinating care, engaging patients and families, and driving program outcomes within a multidisciplinary team.

The ideal candidate has an RN license, BSN preferred with 2 years of RN experience, and case management exposure. Strong communication and data reporting skills are required, with occasional non-standard hours expected.

Qualifications

• Graduation from an accredited School of Nursing.
• BSN preferred with two years’ experience as a Registered Nurse.
• Case management experience preferred.
• Bilingual preference with fluency in Spanish preferred.

Responsibilities

• Ongoing evaluation and quality improvement of the Navigator Program and tracking of activity.
• Ensure quality patient care by adherence to policies and effective communication across the continuum of care.
• Identify barriers to patient care and develop strategies to remove them.
• Implement outreach to establish and maintain relationships with community agencies.
• Collect and maintain up-to-date program data and deliver reports showing outcomes and performance improvement.
