WakeMed Health & Hospitals is seeking a Nurse Navigator, RN, to guide patients through care pathways, coordinating disciplines and supporting families. The role emphasizes education, advocacy, and seamless communication across teams to optimize patient outcomes.

The successful candidate will have a Bachelor’s in Nursing (Master’s preferred) and at least 5 years of acute care nursing experience, with strong patient/family engagement and resource linkage.

Qualifications

• Must be a registered nurse with valid licensure.
• Bachelor's Degree in Nursing required; Master's preferred.
• Minimum 5 years of nursing experience in acute care.

Responsabilités

• Coordinate patient care across all disciplines.
