Job summary

Atrium Health Wake Forest Baptist is seeking an RN Navigator at the Spine Center: Clemmons Med Plz. This full-time role is 40 hours per week, Monday–Friday 0800–1700, with no weekends or holidays.

Responsibilities include evaluating and improving the Navigator Program, ensuring high-quality patient care, conducting outreach to community agencies, and tracking program data to demonstrate outcomes. Spanish bilingual preferred; BSN preferred with two years RN experience.

Qualifications

• Graduation from an accredited School of Nursing is required.
• BSN preferred with two years' RN experience.
• Case management experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Evaluate and improve the Navigator Program and track activity per standards.
• Ensure quality patient care by communicating plans and addressing barriers across care continuum.
• Perform outreach to establish and maintain relationships with community agencies.
• Collect and maintain program data and deliver reports showing outcomes and improvement.
• Trained in motivational interviewing and health coaching for chronic care management.
• Participate in shared governance and pursue ongoing professional development.
