Job summary

Atrium Health Wake Forest Baptist is seeking an experienced RN Patient Navigator to guide patients through the healthcare system, coordinating care from diagnosis through survivorship. You will establish contact with patients, families, and caregivers, identify barriers to timely care, and connect patients with services in our organization and the community.

This role requires a BSN, NC RN licensure, and 2+ years of nursing experience in relevant specialty; bilingual in Spanish preferred.

Qualifications

• Graduate from an accredited school of nursing; BSN required.
• Minimum two years' experience as a registered nurse in the specialty.
• 3-5 years of nursing experience strongly preferred.
• Current licensure to practice as Registered Nurse in North Carolina.
• Certification as an RN preferred; BCLS required.

Responsibilities

• Use nursing process to provide patient/family centered care.
• Coordinate delivery of patient care and improve outcomes.
• Document per standards and generate required program reports.
• Outreach to community agencies and serve as liaison in underserved communities.
• Participate in governance, peer review, and professional development.
