Job summary

Atrium Health Wake Forest Baptist is seeking an experienced RN Patient Navigator to guide patients through the healthcare system and coordinate care across services. This role requires a BSN, NC RN license, and 2+ years of RN experience with a strong preference for 3-5 years in the specialty.

Compensation includes comprehensive benefits, retirement contributions, and education support. The position involves travel to market locations as needed and a rotating call schedule.

Qualifications

• Graduate from an accredited school of nursing, BSN is required.
• Minimum two years' RN experience in the specialty is required.
• 3-5 years of nursing experience strongly preferred.
• Current licensure to practice as RN in North Carolina.
• Certification as an RN preferred and BCLS required.

Responsibilities

• Guide patients through the healthcare system, directing to services for timely diagnosis and survivorship.
• Identify and address barriers to care to ensure timely treatment.
• Support transition from active treatment to continuing healthcare.
• Utilize nursing process to provide patient/family centered care.
• Coordinate care and document per department standards.
• Develop reports and participate in quality improvement activities.
