• Graduation from an accredited School of Nursing.
• BSN preferred with two years' experience as a Registered Nurse.

Responsibilities

• Evaluate and improve the Navigator Program and track activity per standards.
• Ensure quality patient care by collaborating with patients and families and guiding the care plan.
• Identify and remove barriers to efficient patient care; liaise with community agencies.
• Collect, maintain, and report program data and outcomes; meet productivity benchmarks.
• Provide chronic care management with motivational interviewing and health coaching.
