Mercyhealth Hospital and Trauma Center - Janesville, WI is seeking a Registered Nurse Patient Navigator for case management. The role coordinates patient care across settings to ensure appropriate use of resources and improve outcomes.

The position is 40 hours per week, Monday to Friday, with day shifts. Requires BSN or enrollment in BSN completion, RN licensure in Wisconsin, and knowledge of Allscripts and Epic EMR systems. BLS/CPR within 90 days is required.

Qualifications

• Graduate of an accredited nursing program.
• BSN required or enrollment in an accredited BSN Completion Program.
• Case management certification preferred.
• BLS/CPR certification within 90 days.

Responsibilities

• Provides case management, advocacy, and care coordination to ensure seamless transitions and reduce readmission risk.
• Performs early patient assessment to identify coordination and discharge planning needs.
• Creates individualized, patient-focused care plans for ongoing needs.
• Places referrals to post-hospital providers using community resources.
• Collaborates with the care team to ensure timely discharge, education, and follow-up.
