CHS Corporate in Tucson, AZ is seeking a Patient Navigator - RN to guide patients and families through the care continuum. You will coordinate care, address barriers to treatment, and educate patients while collaborating with interdisciplinary teams to ensure a seamless, patient-centered experience.

Ideal candidates have 3–5 years of nursing experience and 1–3 years in navigation or care coordination, with RN licensure and BCLS.

Qualifications

• 3–5 years of clinical nursing experience required.
• 1–3 years in patient navigation, care coordination, or case management preferred.
• RN licensure and BCLS certification required.

Responsibilities

• Serves as primary point of contact for patients and families from diagnosis through treatment.
• Assesses medical, emotional, and socioeconomic needs; coordinates solutions to enhance adherence.
• Provides patient education on treatment, side effects, and care strategies.
• Facilitates communication between patients, families and providers to share information.
• Acts as patient advocate, coordinating with multidisciplinary teams to improve services and reduce delays.
• Coordinates referrals to social workers, financial counselors, dietitians, and community resources.
• Assists with insurance matters, including forms and financial assistance.
• Develops and updates educational materials for patients and caregivers.
• Tracks patient progress across the care continuum and adjusts plans as needed.
