Job summary

United Health Centers is recruiting a PACE Registered Nurse (RN) to support our new PACE site, delivering clinical care and intake assessments to help seniors stay at home.

Under the direction of primary care, you will assess, coordinate, and provide treatment, participate in the interdisciplinary team, and supervise medication administration and wound care. CA RN license and ACLS certification required; bilingual preferred.

Qualifications

Job description

We are recruiting for a PACE Registered Nurse (RN) to join our new Program of All-Inclusive Care for the Elderly (PACE) location. PACE provides comprehensive medical and social services to help older adults remain in their homes instead of moving to a nursing home. We are looking for mission-driven professionals who believe that comprehensive, community-based care is the future of senior health.

Under the direction of primary care, this position is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by PACE members including intake assessments for potential enrollment into the UHC PACE program.

PERFORMANCE AREA 1

• Provide care to UHC PACE participants within scope of licensure understanding the PACE model of care and the individual participant needs.
• Receive and communicate appropriate information to staff and keeps PACE Director informed of changes, needs, complaints, etc.
• Collaborate with IDT to ensure participant’s needs are being met, care plan is being followed and timely communication is prioritized for the good of the participant.
• Responds to emergencies involving participants.
• Assists in the implementation of new procedures.
• Performs functions related to clinical assessment, care planning, evaluation of participant care.
• Teaches participants and families based on identified needs of participant.
• Documents nursing care in the medical record timely and completely.
• Able to triage participants in clinic effectively.
• On call responsibilities.
• Administers medications and maintains medication system including over the counter, contingency and emergency medication supply participants under clinic standards of practice and within scope of licensure.
• Completes transcription and follow-up of medical orders.
• Performs phlebotomy procedures as needed.
• Monitors all logs required in the Health Center to ensure completion and equipment is in working order.
• Maintains cleanliness and organization in the clinic, especially in the medication area and cart.
• Participates and functions as a member of the interdisciplinary team, attending meetings and rotates as needed.
• Independently delivers nursing care and services to program members within scope of licensure and per primary care orders or nursing care plan.
• Schedules and coordinates med pass and monitors for quality review of medication errors.
• Mentors and assists in the orientation of new clinic staff hires.
• In collaboration with the purchasing agent, oversees medical, pharmaceutical and oxygen supplies in the clinic. Tracks assignment of oxygen concentrators and other Durable Medical Equipment as indicated.
• Scheduling and managing appointments/assessments for MDs and Nurse Practitioners
• Wound care and maintaining wound records/documentation
• Daily nursing tasks, preparing clinic for patients to be seen by provider.
• Assist with bedside procedure including gynecological exams, phlebotomy.
• Maintaining participant safety while at the center
• Act as liaison between providers/family/pharmacy and will be an active member of interdisciplinary team and cross train to home care RN position if needed.
• Supports agency’s mission and values.
• Performs related duties as assigned.

PERFORMANCE AREA 2

Focus on Clinicians

• Works in conjunction with the Medical Reception staff for walk-in participant (i.e., obtains and documents participant chief complaint and presents to provider to determine if participant is to be seen or needs to be referred out).
• Helps the providers maintain pace by keeping them informed of participants that are ready.
• Prepares and sets up participants for exams according to providers' expectations.
• Alerts clinicians of possible complicated visits so they are prepared.
• Assists providers when assistance is needed.
• Helps participants by providing basic knowledge of types of services provided by outside facilities.

QUALIFICATION REQUIREMENTS

Education

Licensed as a Registered Nurse in the state of CA.

ACLS certification required.

Experience

• One-year experience as a Registered Nurse; experience with an elderly population preferred

The hourly wage range that UHC reasonably expects to pay for the position upon hire starts at $42.87 an hour. Our salaries are dependent on knowledge, skills, and experience.
