San Ysidro Health is seeking a Registered Nurse to contribute as an interdisciplinary team member, delivering high-quality care to PACE participants. The role requires independent nursing judgment, collaboration with multiple disciplines, and responsibility for managing complex clinical situations across care settings.

The RN will assess and plan care, implement treatments, educate participants and families, and coordinate care with the IDT while ensuring patient safety and adherence to PACE

Competenze

• Graduate of an accredited school of nursing.
• Current license to practice as a Registered Nurse in California.
• Current BLS (American Heart Association)

Mansioni

• Provide direct and indirect nursing care to SD PACE participants.
• Assess, plan, implement, and evaluate patient care within the IDT.
• Coordinate with other health professionals and travel to various sites.
• Document care in EPIC and maintain compliance with regulations.
