Job summary

Family HealthCare Network is seeking a PACE Community Outreach Specialist in California to lead outreach, enrollment, and enrollment growth for PACE participants. You will coordinate marketing campaigns, publications, and community events to raise program awareness across providers and the public.

The role requires strong communication, two years in outreach or related fields, and knowledge of health services and grant-funded initiatives.

Qualifications

• Two years of community outreach, educational activities, and customer service experience (preferred).
• Two years of research and experience developing/promoting public information campaigns and events.

Responsibilities

• Recruit and enroll PACE participants; grow and maintain enrollee census.
• Plan, coordinate, and evaluate marketing strategies for recruitment and retention.
• Coordinate publications, media, and promotional materials with design and content.
• Develop and maintain public relations contacts; draft press releases for approval.
• Coordinate events, ceremonies, and community presentations; provide health education.
• Support data collection and reporting for program activities and grant initiatives.
• Ensure attendance policies are followed and provide outreach across FHCN locations.
