Qualifications

• Completion of RN program required.
• Graduate with a Bachelor or Associate Degree in Nursing.
• Minimum two years' experience in Care Management, Primary Care, or Home care.
• Experience with older adults and families; PACE center preferred.
• Bilingual (Spanish) preferred.

Responsibilities
