Job summary

Peak Rehabilitation Hospital is seeking a Registered Nurse (RN) Outcomes Manager to support CMS IRF-PAI processes and interdisciplinary care coordination. The role focuses on accurate data collection, timely admissions/discharges, and maintaining QA-driven documentation.

You will work in a collaborative environment with Lifepoint Health partners to enhance patient outcomes and ensure compliance with established guidelines. Schedule: FT Days, 8a-4p, Monday through Friday.

Qualifications

• Must hold current North Carolina or Compact RN license.
• ADN from an accredited nursing program.
• CPR/BLS certification required; ACLS preferred.
• QI competency and familiarity with healthcare workflows.
• Strong data entry and documentation skills.

Responsibilities

• Coordinate PPD data collection with interdisciplinary team.
• Ensure documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS timeframes.
• Abstract information from medical records and summarize IRF-PAI.
• Ensure IRF-PAI completion within 4 days of admission and 5 days of discharge.
• Enter data into CMS-accepted software within designated timeframes.
• Maintain and update the IRF-PAI log with accuracy.
• Assist with team education on QIs and documentation standards.
