Job summary

Lifepoint Health® UW Health Rehabilitation Hospital – Madison seeks an Outcomes Manager to coordinate IRF-PAI data collection, CMS compliance, and QI documentation across the interdisciplinary team to support timely and accurate reporting.

You will ensure admission and discharge IRF-PAIs are completed within CMS timeframes, maintain logs, and educate staff on data standards, contributing to high-quality patient care and regulatory readiness.

Qualifications

• Proven ability to collect and manage IRF-PAI and QI data accurately.
• Strong understanding of CMS requirements and facility policies.
• Excellent communication and teamwork across disciplines.

Responsibilities

• Coordinate IRF-PAI data collection with the interdisciplinary team.
• Ensure documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS timeframes.
• Abstract information from medical records and summarize on the IRF-PAI.
• Adhere to four-day admission and five-day discharge deadlines for IRF-PAI.
• Enter data into CMS-approved software within designated timeframes.
• Maintain IRF-PAI logs and verify accuracy with the roster.
• Collaborate across disciplines to complete IRF-PAI logs.
• Educate team on QIs and documentation standards; support QI competencies.
• Perform other duties as assigned.
