Job summary

Peak Rehabilitation Hospital, part of Lifepoint Health, is seeking a Registered Nurse (RN) Outcomes Manager to lead data collection, documentation, and IRF-PAI submissions within CMS timelines. The role emphasizes interdisciplinary collaboration, accuracy, and ongoing QI education.

Ideal candidates hold an ADN, NC license, CPR/BLS, with ACLS preferred. The position offers a Monday–Friday day shift in Apex, NC, and comprehensive benefits aligned with a hospital network committed to community

Qualifications

• Requires NC or Compact RN license.
• ADN from accredited nursing school.
• CPR/BLS certification required; ACLS preferred.
• QI competency; knowledge of healthcare workflows and documentation.
• Basic computer and data entry proficiency; strong clinical assessment and ability to meet deadlines.

Responsibilities

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure complete documentation for QIs in the medical record.
• Complete admission and discharge IRF-PAIs within CMS timeframes.
• Abstract information from medical records and summarize on the IRF-PAI.
• Ensure IRF-PAI completion within four days of admission and five days of discharge.
• Enter data into CMS-accepted software within designated timeframes.
• Maintain the IRF-PAI log and verify accuracy against the patient roster.
• Collaborate across disciplines to complete IRF-PAI fields.
• Assist with team education related to QIs and documentation standards.

Peak Rehabilitation Hospitalis a 52-bed inpatient acute rehabilitation hospital serving the Raleigh/Durham and Apex communities. Operated in partnership with Lifepoint Health, Duke Health, and WakeMed, our interdisciplinary team is dedicated to helping patients regain independence and improve their quality of life in a collaborative, patient-centered environment.

How you'll contribute

A Registered Nurse (RN) Outcomes Manager who excels in this role will:

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure supportive documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
• Abstract required information from medical records and summarize on the IRF-PAI.
• Ensure admission IRF-PAI completion within four (4) days of admission.
• Ensure discharge IRF-PAI completion within five (5) days of discharge.
• Enter data into the CMS-accepted software/program within designated timeframes.
• Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.
• Ensure all columns/fields of the IRF-PAI log are completed in collaboration with each discipline.
• Assist with team education related to QIs and documentation standards; complete and help administer QI competencies for team members.
• Perform other duties as assigned.

What we're looking for

Applicants should have a current North Carolina or Compact State RN license and possess an Associate's Degree in Nursing (ADN) from an accredited nursing school. Additional requirements include CPR/BLS certification required:

• CPR/BLS certification required
• ACLS certification preferred
• Ability to work effectively in a collaborative team environmentClinical degree with current license to practice as an OT, PT, RN, SLP, or MSW.
• QI competency; working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation.
• Basic computer and data entry proficiency; strong clinical assessment/analysis; effective verbal and written communication; excellent interpersonal skills; ability to collaborate across disciplines and meet strict deadlines with high accuracy.

Schedule: FT Days 8a - 4p Monday through Friday
