Job summary

Eastern Iowa Rehabilitation Hospital, part of Lifepoint Health, seeks an Outcomes Manager to coordinate IRF-PAI data collection, ensure CMS compliance, and support QI initiatives.

You’ll partner with nursing, therapy, case management, and medical staff to guarantee timely, accurate data capture and high-quality documentation, contributing to improved patient care.

Qualifications

• Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW.
• Working knowledge of state and federal guidelines; healthcare workflows and documentation.
• Basic computer and data entry proficiency; strong clinical assessment and analysis.

Responsibilities

• Coordinates collection, documentation, and submission of IRF-PAI and QIs data per CMS and facility policies.
• Partners with the interdisciplinary team to ensure timely, accurate data capture and education on QI documentation.
• Works with nursing, therapy, case management and medical staff to support accurate QI scoring and IRF-PAI completion.
• Adheres to CMS regulations, payer guidelines, and organizational data integrity policies.
• May assist with audits, performance improvement activities, and data validation.
