Job summary

PeaceHealth Springfield Rehabilitation Hospital is opening a brand-new inpatient rehabilitation facility in 2026. We are seeking an Outcomes Manager who coordinates PPD data collection with the interdisciplinary team and ensures documentation for QIs in the medical record.

You will complete admission and discharge IRF-PAIs within CMS timeframes, abstract required information from records, and enter data into CMS-approved software while maintaining accurate IRF-PAI logs.

Qualifications

• Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW.
• QI competency required.
• Working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation.
• Basic computer and data entry proficiency.
• Strong clinical assessment/analysis, effective verbal and written communication, teamwork across disciplines.

Responsibilities

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure supportive documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
• Abstract required information from medical records and summarize on the IRF-PAI.
• Enter data into the CMS-accepted software/program within designated timeframes.
• Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.
