Job summary

Peak Rehabilitation Hospital, part of Lifepoint Health, is seeking a Registered Nurse (RN) Outcomes Manager to lead data collection, documentation, and IRF-PAI submissions within CMS timelines. The role emphasizes interdisciplinary collaboration, accuracy, and ongoing QI education.

Ideal candidates hold an ADN, NC license, CPR/BLS, with ACLS preferred. The position offers a Monday–Friday day shift in Apex, NC, and comprehensive benefits aligned with a hospital network committed to community

Qualifications

• Requires NC or Compact RN license.
• ADN from accredited nursing school.
• CPR/BLS certification required; ACLS preferred.
• QI competency; knowledge of healthcare workflows and documentation.
• Basic computer and data entry proficiency; strong clinical assessment and ability to meet deadlines.

Responsibilities

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure complete documentation for QIs in the medical record.
• Complete admission and discharge IRF-PAIs within CMS timeframes.
• Abstract information from medical records and summarize on the IRF-PAI.
• Ensure IRF-PAI completion within four days of admission and five days of discharge.
• Enter data into CMS-accepted software within designated timeframes.
• Maintain the IRF-PAI log and verify accuracy against the patient roster.
• Collaborate across disciplines to complete IRF-PAI fields.
• Assist with team education related to QIs and documentation standards.
