Job summary

Majestic Care of Hopemont is seeking an MDS Coordinator (RN) to supervise MDS and care plans for residents, ensuring timely, accurate assessments in line with state and federal requirements.

Candidates must hold an active RN or LPN license and have LTAC experience; strong data entry/word processing skills and knowledge of MDS 3.0 are required; CPR must be obtained within 30 days. A $10k sign-on bonus is offered along with Majestic Difference benefits.

Qualifications

• Must hold active RN or LPN nursing license in the state of employment.
• Experience in Long Term Care preferred.
• Computer skills such as Data Entry and Word processing required.
• Working knowledge of the MDS 3.0 (current version of minimum data set).
• Must possess the ability to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures for quality care.
• Must hold an active CPR license or the ability to obtain within the first 30-days of employment; CPR license must be kept active and current throughout employment.

Responsibilities

• Coordinate the MDS and Care Plan per facility policies.
• Inform care team when a care plan is due and ensure completion of MDS portions.
• Develop or revise resident care plans quarterly or with significant changes.
• Ensure PPS and state MDS assessments are completed and submitted.
• Coordinate care plan meetings and encourage team participation.
• Maintain timely submission of MDS to state/federal agencies per guidelines.
• Observe residents' rights in the MDS process and maintain confidentiality.
