Job summary

Majestic Care Hopemont in Kentucky is seeking an experienced MDS Coordinator (RN) to oversee MDS assessments and care plans for residents in a timely, compliant manner. You will coordinate MDS timelines, ensure accuracy, and collaborate with the care team to support quality outcomes.

Qualified candidates hold an active RN license in Kentucky, with LTC experience and proficiency in MDS 3.0, data entry, and nursing regulations.

Qualifications

• Must hold current RN nursing license in the state of employment; active and in good standing.
• Experience in Long Term Care preferred.
• Computer skills such as Data Entry and Word processing; Working knowledge of the MDS 3.0.
• Knowledge of nursing practices and regulations for skilled nursing facilities.
• Ability to plan, organize, develop, implement and interpret programs for quality care.
• Must hold an active CPR license or obtain within 30 days.

Responsibilities

• Coordinate the MDS and Care Plan per facility policies.
• Inform care team when a care plan is due and ensure sections are completed.
• Develop or revise resident care plans quarterly or with significant change.
• Manage PPS and state MDS assessments; ensure MDS, CATs, and CAAS completed.
• Coordinate all care plan meetings and encourage team participation.
• Submit MDS to state/federal agencies per Medicare/Medicaid guidelines.
• Maintain resident rights and confidentiality in the MDS process.
• Ensure inter-department communication; adjust during condition changes.
• Monitor MDS process and report concerns to Executive Director and DON.
• Assist with pre-admission screening to estimate RUGs.
