Job summary

Prime Healthcare is seeking an experienced RN or LPN MDS Coordinator to join our skilled nursing team in Tennessee. This full-time role focuses on accurate RAI/MDS assessments, CAAs, and care plans, with no on-call shifts. Qualifications

• Requires active, unrestricted RN or LPN license in Tennessee.
• Long-Term Care / Skilled Nursing experience preferred.
• MDS knowledge or willingness to learn the MDS/RAI process.

Responsibilities

• Coordinate and oversee the RAI/MDS process with accurate, timely submissions.
• Ensure compliance with state and federal regulations and facility policies.
• Develop individualized resident care plans with short- and long-term goals.
• Collaborate with interdisciplinary teams to support care planning and quality measures.
• Monitor completion of resident assessments and submit MDS data as required.
• Facilitate care plan conferences and contribute to QM meetings.
