Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to relocate to New Mexico and support multiple facilities in Alamagordo, Roswell, Gallup, Farmington, Taos, Las Cruces, Grants. You will oversee MDS/RAI processes from admission to discharge under a DON/VP guidance.

The role requires RN licensure, knowledge of Case-Mix/Medicare PPS, and strong documentation practices to ensure compliance with OBRA and state/federal rules.

Qualifications

• Graduate of an approved RN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the OBRA regulations and Minimum Data Set.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure decisions to complete or not complete MDS based on clinical criteria and RAI User’s Manual.
• Assist with scheduling and the resident care planning conferences.
• Maintain documentation to support MDS coding and compliance with regulations.
• Maintain MDS status and data entry in appropriate computerized programs.
• Attend interdisciplinary team meetings and communicate changes to staff and families.
