Job summary

St. Sophia Health & Rehabilitation Center in Missouri seeks an experienced MDS Coordinator (RN) to lead the RAI/MDS process, ensure regulatory compliance, and educate the interdisciplinary team. You will oversee documentation quality, support billing and reimbursement, and drive improvements in resident care planning.

Qualifications

• Graduate of an accredited school of nursing.
• Must be an RN with a current, active Missouri license.
• CPR Certified.
• MDS experience is preferred, will educate candidates to grow their skill set.

Responsibilities

• Understand regulations and standards related to the RAI process.
• Participate in pre-admission processes to obtain MDS/Case Mix data.
• Collaborate with Director of Nursing to ensure timely nursing documentation.
• Attend community and corporate meetings as a MHDS liaison.
• Prepare and submit ADRs/Revenue Audits as required.
• Serve as community subject matter expert on ADL documentation and MDS support.
• Train new associates on the RAI process and educate staff.
• Assess staff competency and educate the IDT as needed.
• Generate clinical, quality and reimbursement reports for review.
• Review quarterly Medicaid reports and take follow-up actions.
