Southview Acres Healthcare Center in Saint Paul, MN, is seeking an experienced MDS Coordinator (RN) to manage the RAI process and ensure accurate reimbursement while supporting high-quality resident care.

The role requires coordination with the interdisciplinary team, strong knowledge of MDS, RAI, ICD-10 coding, and PDPM, and a solid background in SNF/long-term care settings. Excellent attention to detail and communication are essential.

Qualifications

• Current RN license in Minnesota.
• Minimum 1–2 years of MDS experience preferred.
• Strong knowledge of RAI process, ICD-10 coding, and PDPM.
• Experience in a skilled nursing facility (SNF) or long-term care setting.
• Strong organizational skills and attention to detail.
• Excellent communication and teamwork abilities.

Responsibilities

• Coordinate and complete MDS assessments (OBRA, PPS, and state-specific assessments).
• Ensure accuracy and timeliness of all MDS documentation and submissions.
• Collaborate with interdisciplinary team members to develop and update comprehensive care plans.
• Monitor case mix index (CMI) and identify opportunities for optimization.
• Ensure compliance with CMS guidelines and Minnesota state regulations.
• Participate in care plan meetings and communicate with residents and families.
• Assist with audits, surveys, and quality improvement initiatives.
• Stay up to date on regulatory changes and reimbursement updates.
