Job summary

River Towne Care and Rehabilitation Center in Columbus, GA is seeking an experienced MDS Coordinator to manage the resident assessment process and ensure compliance with federal and state regulations, including Medicare/Medicaid reimbursement guidelines.

This full-time position requires an active RN license, at least 2 years in long-term care with MDS knowledge, EMR proficiency, and strong leadership and communication skills. Hours are Mon-Fri 8am-4pm with negotiable salary.

Qualifications

• Must be an RN in good standing with state board.
• Active RN license required.
• Minimum 2 years nursing experience in long-term care or rehabilitation.
• Knowledge of CMS regulations (RUG-IV/PDPM).
• Proficient in MDS, EMR, and documentation systems.
• Strong organizational and communication skills.
• Leadership and collaboration with interdisciplinary teams.

Responsibilities

• Complete and submit MDS 3.0 assessments per CMS guidelines.
• Coordinate OBRA and PPS assessments.
• Maintain accuracy of MDS data for all residents.
• Audit charts and MDS records for timeliness and accuracy.
• Educate staff on MDS processes and documentation.
