River Garden Senior Services - River Garden Hebrew Home For The Aged is seeking an experienced MDS Coordinator to join our geriatric care team. The role focuses on directing MDS coding, care plans, and PDPM processes, with emphasis on leadership and interdisciplinary collaboration in a nonprofit long-term care setting.

Two years of MDS coding experience and an active Florida RN license are required; RNAC is preferred.

Qualifications

• Two years of experience in MDS Coding/Dx Coding, PDPM, Triple Check and Med Certifications.
• RNAC Certification preferred.
• Two years of recent clinical experience in a health care setting, geriatric setting preferred, one year supervisory or management experience.
• An active RN License within the state of Florida.

Responsibilities

• Care plan resident needs as assessed by care team.
• Coordinate and oversee all areas of the MDS assessment process to assure compliance with all federal and state requirements.
• Participates in the management of resident care
• Demonstrates appropriate and safe work practices and behaviors.
• Identify changes in resident’s status and intervenes based on nursing knowledge and judgment as well as MD orders.

What We Are Looking For:
