Job summary
Harborview Health Center of Augusta in Georgia seeks an experienced MDS Coordinator (RN) to lead the MDS assessment process, optimize reimbursement, and ensure CMS compliance. You will collaborate with Nursing, Therapy, Social Services, and other teams to develop resident-centered care plans.
The role requires active RN license, experience in skilled nursing, and familiarity with MDS tools such as SimpleLTC and PCC. We offer top pay and a comprehensive benefits package.
Qualifications
- Active RN license in good standing in Georgia.
- Experience in skilled nursing and long term care.
- Experience using MDS tracking and assessment tools such as PCC/SimpleLTC is a plus.
Responsibilities
- Lead and submit MDS assessments on time with accurate data.
- Select ARDs and codes to ensure PDPM accuracy and reimbursement.
- Monitor PDPM components and validate documentation for CMS compliance.
- Coordinate care planning across Nursing, Therapy, Social Services, Dietary, and Activities.
- Lead IDT meetings: daily PDPM, weekly CMI, Utilization Review, Triple Check, and care planning conferences.
- Utilize tools like PCC, SimpleLTC, NetHealth, Teams, IQIES, Power DMS, and Google Sheets.
